 MEDICAL CASE DISCUSSION



This is an E log book to discuss our patient's de-identified health data shared after taking his guardian's signed informed consent. Here we discuss our individual patient problems through series of inputs from available global online community of experts with an aim to solve those patient's clinical problems with collective current best evidence based inputs. CASE DISCUSSION

A 70 year old Male came to GM OPD with chief complaints of 

  1. Fever since 14 days
  2. Shortness of breath since 7 days 
  3. Cough since 7 days
  4. Generalized body pains since 7 days 

HISTORY OF PRESENTING ILLNESS

The patient was apparently asymptomatic 30 days back then he developed
  • Fever which was insidious in onset and of intermittent type, not associated with chills/rigor, no aggrevating factors present but relieved with medication.
  • Dry cough which was insidious in onset and has no aggrevating or relieving factors.
  • Loss of appetite
  • Generalised body pains

He was tested POSITIVE for SARS COV-2 on 20th of April 2021.

He was admitted to Hospital -1 and got treated for SARS COV-2.

Got tested again for SARS COV-2 on 1st May 2021 (in govt hospital) results were NEGETIVE

But the patient was still having symptoms 
So he got HRCT done in another hospital and was declared POSITIVE for SARS COV-2 by assessing HRCT.

The patient was in HOME ISOLATION as his saturation levels were normal.

From past 7 days he developed shortness of breath and it aggrevated on 18th May 2021 and the patient was admitted in the present Hospital.


PAST HISTORY

  1. Patient is a k/c/o - Psoriosis since 15 years
  2. k/c/o - DM type 2 since 4 years
  3. k/c/o - HTN since 4 years
  4. He got a HEART ATTACK 4 years back and underwent surgery for the same.
FAMILY HISTORY

His Son was also tested POSITIVE for SARS COV-2 on 20th April 2021.
He was in HOME ISOLATION and took the general medication.
Got tested again on 1st May 2021 and the results were NEGETIVE.

PERSONAL HISTORY

Diet                   - Mixed
Sleep                - Adequate
Appetite            - Decreased
Bowl & Bladder - Normal
Addictions         - Regular intake of alcohol (90ml) from 20 years
                          - Before used to consume Toddy for 10 years

SURGICAL HISTORY

CABG(coronary artery bypass graft) - 4 years back

GENERAL EXAMINATON

The patient was conscious, coherent and co-operative.
He is well oriented to time place and person.
